Confidential Incident Report

Any kind of harassment or aggression, major or micro, is a serious concern to our chemistry department. If you find yourself in a situation outside the normal expectations of a friendly, inclusive, professional, safe and supportive environment, here is the place to get help. 

The contents of your report will be confidentially sent to faculty on the Chemistry Diversity, Equity and Inclusion Committee (Claudio Verani, Nancy Wu, James Bour). You may also report incidents directly to the department chair or individual faculty members of the committee.

Once completed, one of the DEI members will contact you as soon as possible. If you wish your report to be kept anonymous, please indicate "I wish this to be an anonymous report" and we will remove your name/AccessID from any further action.
